POSITION SUMMARY

Provides routine daily nursing care and services in accordance with the care plan of each resident based on established nursing care procedures and at the direction of supervisor. Ensures resident’s needs are maintained with highest degree of dignity.

DUTIES AND RESPONSIBILITIES

  1. Demonstrates commitment to company’s mission, values and standards of ethical behavior.
  2. Models a positive and professional image that emulates the organization's mission and encourages a spirit of cooperation with co-workers. Provides assistance to co-workers and promotes teamwork by sharing information useful to achieving desired results
  3. Ensures that all residents are treated fairly, with kindness, dignity, and respect, and their rights are protected at all times.
  4. Respects and upholds all residents’ personal and property rights whenever carrying out duties, including knocking before entering a resident's room.
  5. Complies with all company and departmental policies and procedures.
  6. Performs comprehensive resident care duties including but not limited to bathing, taking vital signs, changing linens, properly positioning residents and giving AM and PM care.
  7. Documents objective information related to resident care in a clear, complete and timely manner.
  8. Promptly answers resident call lights and provides appropriate responses to requests.
  9. Identifies residents by identification band prior to administering treatments, serving meals, etc.
  10. Reviews care plans daily to determine if changes in resident’s daily care routine have been made.
  11. Maintains unit in an orderly fashion. Clears hallways of equipment or positions it on one side.
  12. Prepares residents for meals, serves and picks up food trays and snacks, and assists with feeding as indicated.
  13. Distributes and removes bed pans, urinals and emesis basins as needed.
  14. Collects routine specimens as directed.
  15. Provides assistance with admissions/transfers/discharges including stripping room, completing inventory and marking resident’s personal possessions.
  16. May assist in transporting residents to and from appointments, activities and social programs.
  17. May perform restorative nursing duties.
  18. Reports the following in accordance with established facility procedures and regulatory standards: accidents and incidents; resident grievances, complaints, allegations of resident abuse and/or misappropriation of resident property; occupational exposure to blood, body fluids, infectious materials, and hazardous chemicals; concerns and suspected incidences of non-compliance including but not limited to hazardous conditions and equipment.
  19. Adheres to state and federal regulations and company policies and procedures; HIPAA confidentiality standards of resident and facility information; Infection Control, Universal Precautions, OSHA and safety standards; and established safety precautions and regulations (including use of mechanical lifts when lifting or moving heavy individuals or objects) in the performance of tasks and the use of equipment and supplies.
  20. Maintains safe and clean work area and ensures equipment is cleaned and stored at end of shift.
  21. Attends orientation, in services and meetings as required. Participates in OSHA and CDC in-service training programs for hazardous communication, TB management, and bloodborne pathogens standard. May participate in workshops and seminars to stay current with changes in field, as well as to maintain professional status.
  22. Takes responsibility for staying updated on new regulations, best practices and internal policies and procedures.
  23. Demonstrates knowledge of facility and departmental quality improvement activities. Recommends to Supervisor any equipment and supplies needed for the department as well as any departmental improvements.
  24. Utilizes most efficient, safe and cost effective techniques and methods, and uses supplies and equipment in most efficient manner to minimize waste.
  25. Participates in and assists in departmental studies and projects as required.
  26. Works with facility consultants as necessary and implements recommendations as required.
  27. Interprets department policies and procedures to personnel, residents, visitors, family members, etc., as necessary.
  28. Performs other related duties as required.

KNOWLEDGE, SKILLS, ABILITIES & QUALIFICATIONS

  1. Previous Nursing Assistant experience providing direct care to multiple patients/residents preferred; previous experience in Skilled Nursing environment desirable
  2. Completion of certification program for Nursing Assistants and maintenance of current unencumbered certification required.
  3. Current valid CPR certification strongly preferred
  4. High school diploma or equivalent required

  9. Ability to understand and follow applicable regulations, policies and procedures fully and consistently
  10. Willingness and ability to function as an effective team member through good communication and cooperation to meet departmental goals
  11. Must demonstrate flexibility and ability to adapt to change
  12. Ability to organize, prioritize and meet deadlines
  13. Ability to maintain confidentiality and be discreet
  14. Must be able to multi-task and handle pressure.
  15. Must be able to follow instructions and to accept feedback
  16. Must successfully pass background check, health exams, and any other pre-employment requirements
  17. Must be able to operate departmental equipment, as appropriate
  18. Willing to work beyond normal working hours, weekends, holidays, and/or on other shifts and in other positions, when necessary
  19. Must be knowledgeable of nursing/medical practices and procedures, and/or terminology, laws, regulations, and the guidelines that pertain to long-term care
  20. Must be able to read, write and speak the English language sufficiently and comprehensibly enough to perform the essential functions of the job.
